Day Kimball Healthcare offers outpatient nutrition education and counseling for all ages to support you as you make positive behavioral changes to improve your health and meet your dietary needs. 

About the Program

Nutrition Education is offered by our nutritionist who is a credentialed registered dietitian (RD) and receives ongoing education in the latest research-based treatments. The RD is there to guide you through your nutrition and wellness journey by working together to identify areas of concern, create behavior change goals, and monitor your progress.

We provide one-on-one counseling and work closely with you to develop a customized plan to help you meet your unique health goals.

Cost

Nutrition Education covered by most insurance groups including Medicare. Please confirm with your insurance provider to verify your benefits before scheduling an appointment.

Referrals

Consultations are available for children and adults at our Day Kimball Healthcare Center in Plainfield. A referral from your primary care practitioner (PCP) is required. Ask your provider for a referral today.

About Our Registered Dietitian

Sarah Balko, RD, CDCES, is a registered dietitian and certified diabetes educator in the Day Kimball Medical Group's endocrinology department at Day Kimball Healthcare. She has been educating patients in improving their health for 20 years. She treats both children and adults, and her specialities include medical nutrition therapy and diabetes education. Billing for nutrition education services will come from Day Kimball Medical Group. Please check with your insurance carrier for details about your specific coverage, copays, deductibles and any prior authorization requirements.
